Payment for 100% of your child’s tuition is due with your completed registration. A non‐refundable deposit of 50% of this amount will be kept by The Rabbit Hole in the event of cancellation before your child’s class start date. After this date, no refunds will be made.

Make-up classes can be set with your teacher on a per-case basis.

I hereby authorize the staff of The Rabbit Hole to administer appropriate medical attention to my child in the event of any accident, illness or injury. I will be responsible for any or all cost of medical coverage and treatment provided, not covered by insurance. I know of no physical, emotional, or injury related conditions which will be aggravated by or which may affect my child’s ability to participate.

I hereby grant The Rabbit Hole permission to use my family’s likeness in a photograph in any and all of its publications, including website entries, without payment or any other consideration. I understand and agree that these materials will become the property of The Rabbit Hole and will not be returned. I hereby irrevocably authorize The Rabbit Hole to edit, alter, copy, exhibit publish or distribute this photo for the purposes of publicizing The Rabbit Hole’s programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein our likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of photography. I hereby hold harmless and release and forever discharge The Rabbit Hole from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
